Here are steps how to fill out the MD office enrollment form:

  1. Users who have to consult or admitting privileges at Sentara hospitals.
  2. Refer patients to Sentara hospitals for labs, emergency, or outpatient diagnostic service.

The next sections of this form consist of 5 parts. Here is information what you need to provide in each part of the form:

Part 1 – Provide user login information. In this part, you must enter first and last name, middle initial, Suffix (Jr, Sr, etc.), date of your birth, gender, SSN, pager number, business number, and email address.

Part 2 – This part involves Part 2A, and Part 2B sections. In each part, you should provide information about Billing or Practice Company Demographic details. Enter the full name of the Billing or Practice Company, address, city, state and zip code, telephone number, tax ID number, and vendor numbers.

Part 3 – Role information. Here, you must tick the one appropriate role out of this list:

  • Physician
  • PA/NP
  • Medical Student (1st or 2nd tear)
  • Medical Student (3rd or 4th year)
  • EpicCare Link Administrator.

Part 4 – Now you are asked to tick YES or NO for each of the statements below:

  • The user will request for patients to be scheduled for the Sentara Surgery Suite.
  • The user will reschedule Radiology procedures to be finished for patients at Sentara healthcare facilities.
  • The practice has NOT finished an EpicCare Link conversion (you may check with your Practice Manager).

Part 5 – Practice Supervisor. In this last part of the MD office Sentara physician enrolment form, you will be asked to provide information about the supervisor's name, email address and telephone number. Once you provide all the necessary information in each part and field, you will have to click on the "Submit" button, located at the bottom of the form.
